Below you will find a new patient and consent forms that is required to be filled out and sent back via email before your confirmed appointment.
New Patient Forms
How to download:
Desktop Computer (Windows or macOS)

Open the PDF File:
Use Adobe Acrobat Reader (free) or Adobe Acrobat (paid).Right-click the PDF file, choose "Open with," and select Adobe Acrobat Reader or Adobe Acrobat.

Fill Out the Form:
Click on the form fields and type in your information.
For checkboxes, click on the box to check or uncheck.

Save and Share the PDF:
Once filled, go to "File" > "Save" or "Save As" to save your changes. Send PDF via email to islandsidesurgery@gmail.com

Ensure you remember where you save the file for later retrieval.

iPhone

Open the PDF File:
Use the built-in Files app or a PDF reader app like Adobe Acrobat Reader (free).
Tap on the PDF file to open it.

Fill Out the Form:
Tap on the text fields to enter your information.
For checkboxes, tap them to toggle the check.

Save and Share the PDF:
The PDF is typically saved automatically in the Files app.
Share the PDF via email to islandsidesurgery@gmail.com using the share icon.
Android Phone

Navigate to the PDF file and tap to open it.

Fill Out the Form:

For checkboxes, tap to select or deselect.

Save the PDF:
Most PDF readers save changes automatically.
You can also use the “Save As” option to save a new copy of the filled PDF.

Share:
Use the share option within the app to send the PDF via email to islandsidesurgery@gmail.com
Pre-Operative Instructions
As you prepare for your pet's upcoming surgery at Islandside Surgical Center, please follow these important pre-operative instructions to ensure a safe and smooth procedure for your beloved companion.
1. Drop-off and Pick-up Times:

Drop-off: Please bring your pet to our center by 7:30 AM on the day of the surgery.
Pick-up: Your pet will be ready to go home at 6:00 PM. Please be prompt in picking up your pet.

2. Fasting Requirements:

No Food After Midnight: It is crucial that your pet does not eat after midnight the night before the surgery. This helps prevent nausea and reduces the risk of aspiration during anesthesia.

Water is Allowed: Your pet can have access to water up until the morning of the surgery.
​
3. Medications:

If your pet is on any medications, please inform us in advance. Depending on the medication, we will advise you on whether to administer it on the day of the surgery.

4. Comfort and Safety:

Please bring your pet on a leash or in a carrier for their safety.
​
5. Post-Operative Care:
-
We will provide you with post-operative care instructions at the time of pick-up. This will include information on medication, activity restrictions, and diet for your pet’s recovery period.

6. Emergency Contact:

Please ensure that we have your current contact information in case we need to reach you during the day.
